Biologic and psychologic factors form the basic for expression of our sexuality. Sexuality is a normal human experience and sexual dysfunction evokes a highly emotional reaction. During a normal sexual response, erection and tumescence of the penis has to be maintained during the excitement, plateau and orgasmic stages. Penile erection has a neurovascular basis involving arterial dilation, sinusoidal relaxation and limitation of venous outflow. Erectile dysfunction is a leading cause of sexual disturbance and one estimate shows that one out of every three patients over 50 years of age who seek medical help for any ailment may have erectile dysfunction. The cause of erectile dysfunction has to be determined in order to undertake the treatment. Though treatment of underlying organic abnormality or psychologic aberration of patients, many need therapy which may be with oral, parenteral or topical drugs. The treatment is individualized and a variety of treatment option are available to the person and his partner. The advantages and disadvantages of various therapeutic modalities have been pointed out. It is hoped that this book will help the practitioners and medical students to adopt a rational outlook on the treatment option for this disturbing sexual disorder characterized by erectile dysfunction. |
